As the parent or guardian of the student named in this form, I undertake to observe the following terms for participation in the After School programme:

Yo, padre/madre o tutor(a) del alumno(a) indicado en este formulario, me comprometo a cumplir con los siguientes lineamientos para la participación en el programa After School:

1. I undertake to settle the monthly fee for each discipline before the 10th of each month so that my child may take part. Otherwise they cannot be received in class and will wait in Reception; I will be notified by telephone to make the payment or to collect the student.
2. I accept that all payments must be made directly at the Cashier or by transfer or deposit to the school's official accounts. If paying by transfer, I undertake to send the receipt to the cashier's email with the student's name, the teacher's name and the discipline.
3. Should the student be unable to continue attending, I undertake to give formal notice of withdrawal in writing or by email before the new month begins.
4. I understand that students who stop attending without having requested withdrawal are considered enrolled, and the regular charge for that month will therefore be raised even if they do not attend. To request withdrawal I will email the After School staff and complete the satisfaction survey.
5. I understand that the school's facilities, resources and materials may only be used during the class's scheduled hours.
6. I accept that materials may not be taken out of the school, nor used for activities outside the programme.
7. I acknowledge that it is the student's responsibility to keep their workspace clean, look after their materials and return them to their proper place; the school is not liable for damage to or loss of property.
8. I understand that I must collect my child punctually after their class, as our after-hours care operates only until 6:00 p.m.
9. I am aware that eating during After School classes is not permitted.
10. I undertake that my child will behave respectfully towards classmates and teachers, both inside and outside the school; failure to observe these rules may lead to permanent withdrawal from the programme.
11. I understand that the school may exercise its right of admission where a student breaches the rules of conduct on more than three occasions, each of which will be notified to the email address provided.
12. I undertake to provide an up-to-date telephone number on which I can readily be reached.
13. I am aware that After School classes are suspended during the December and Holy Week holidays.
14. I know that the school offers two-month packages at a special rate over these periods; if interested, I undertake to request them at the Cashier or from the After School staff before the holiday period begins so that they are applied correctly.

I declare that I have read and understood the terms of this commitment letter and accept each of the points set out in it.
